Who we are

Two women on a mountain hike, smiling and striking playful poses with a scenic mountain landscape in the background.

We are a therapist-led company founded by two college friends with more than 65 years of combined experience in home health and assisted living care. Each founder brings nearly 35 years of clinical experience focused on providing physical therapy within assisted living communities. This depth of experience allows us to understand the unique needs of residents and deliver outpatient therapy that is both clinically sound and well-suited to the assisted living environment.

At the heart of our work is a simple belief: therapy should help people stay strong, confident, and independent. By bringing outpatient physical therapy directly into assisted living facilities, we remove common barriers to care and make it easier for residents to stay active and engaged in their daily lives. After decades of working side by side with residents and care teams, we know how powerful the right support can be—and we’re proud to be part of that journey.

Elevate. Uplift. Arise.

TRACY M. LATTIN

The University of Utah - Physical Therapy ‘92

“I have spent over 34 years specializing in geriatric care, both in assisted living facilities and providing home health care. While studying at the University of Utah, I began my career as a sports trainer from 1988-89, which gave me a strong foundation in movement, strength, and injury prevention. From there, I transitioned into working in assisted living facilities where I’ve been privileged to help residents maintain mobility, independence, and quality of life starting in Dallas, TX and now in Salt Lake City, UT.

Every day, I get to pursue my passion of helping residents elevate their quality of life with clinical and emotional support. I specialize in motivating clients to reach their full potential, combining my expertise with a compassionate, resident-centered approach. I love problem-solving complex mobility challenges and creating comprehensive care plans in collaboration with residents, their families, and care staff. For me, every visit is an opportunity to help someone live more confidently, actively, and independently.”

KATIE W. PETERSON

The University of Utah - Physical Therapy ‘93

“I am a licensed physical therapist with 33 years of experience and a graduate of the University of Utah’s Physical Therapy program. I have practiced in a variety of settings, including orthopedic outpatient clinics, hospitals, home health, and private practice, and have served as both a rehabilitation director and physical therapy coordinator.

This broad background has shaped a practical, hands-on approach focused on restoring movement, improving function, and supporting independence. I believe in meeting patients where they are and tailoring care to their individual needs so they can move more comfortably and confidently in daily life.

For residents of independent and assisted living communities and their families, I provide convenient, high-quality physical therapy in a familiar setting, with an emphasis on safety, mobility, and clear communication.

I am trained in dry needling and skilled in manual therapy, using a combination of techniques to address pain, mobility limitations, and movement dysfunction.”
